According to the maintenance records on site, a cold start of the monitoring <br />panel was conducted on October 29, 2013 and the 87 mechanical leak detector was replaced on December 6, 2013. <br />Permits were not obtained from the EHD for either of these repairs. Emergency repairs relating to UST system <br />monitoring and release detection may be performed by a properly licensed, trained, and certified contractor pursuant to <br />the EHD'S policy and a permit obtained within one business day from the EHD. All other repairs must have a permit <br />submitted to and approved by the EHD prior to beginning work. Immediately obtain a permit from the EHD for the <br />10/29/13 cold start and the 87 leak detector replacement. A "Failure to Obtain a Permit" penalty applies. This is a Class <br />II violation. <br />201 Failed to maintain alarm logs and/or records of follow up actions. Maintenance and monitoring records for the last three <br />years were not found on site. Records of all alarms and the response to these alarms was not found on site. The site is <br />keeping alarm logs, but these logs are incomplete and do not list all of the alarms for the site. The alarm logs for <br />November 2013 through September 2014 only included sensor out alarms on November 14, 15, 16, 17, 19, and 21, and <br />do not state which sensor was in alarm. The other days on the alarm logs are either blank or "all functions normal'. <br />These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all <br />monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual <br />observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other <br />monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and <br />reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site <br />and submit copies to the EHD. This is a repeat violation, Class II. <br />204 Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not <br />available for inspection. A copy of these plans shall be accessible on site at all times. Immediately update the <br />monitoring plans in CERS and retain a current, approved copy on site.
