The permit has been approved with the following conditions: <br />1. Contact the local fire department and the Sal Joaquin Valley Air Pollution Contra <br />District for any applicable permits and mquirements. <br />2. Any changes in information and scope of work mast be submitted to EHD forapproval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Thuy Tran at (209) 468-8257 to arrange the following inspection appointments <br />(a 4B -hour notice is required for inspection appoiritmerds). Plesse be aware that <br />inspections for emergency work must be scheduled within five days of permit approval <br />a. Sensor functionattlt. Functionality of the moritodng system must be ventrad to <br />ensure that monitoring has been restored to meet the requirements of Title 23 <br />California Code of Regulations and the Heath & Safety Core. - <br />b. A copy of the alarm history shall be printed prtorto performing the "cold Starr of the <br />Veeder Root system and made avadabte at the time of EHD fnspedlon. <br />Please be aware that the hcfitty cannot operate the UST system prim to the EHD <br />Inspection and approval. <br />4. Qualified serves technicians most possess or work under the diad and personal <br />supervision of an individual physically presets at the wrolk site who possesses iff of the <br />following: <br />a One of several permissible valid, current contractor's licenses issued by2ho <br />Contractors State License Board (may be issued to the installation company), or a <br />Tank Tesfar°s license issued to the irdvktuat by the State Water Board, and <br />b. A valid, current California UST Service Technician certificate issued by the <br />trternafional Code Council (ICC), and <br />a A valid, current certificate of trainiN from the manufacturer(s) of the conponam(s) <br />being tested, repaired, orservaoed. <br />
