The permit hes been approved with the fotovring conditions: <br />1. Contact the local fire department and the San Joaquin Vary Ak Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing wok Al new addenda supersede the previous ones: <br />3. Sensor functionality- Functionally, of the monitoring system must be verfied to <br />ensure that monitoring has been restated to meet the requirements of TiOa 23 <br />California Code of Regulations, and the Health & Safely Code. <br />b. A copy of the alarm history shall be prided prim to performing the 'cold start" of the <br />Veeder Root system and made available at the time of EHD inspection. <br />Please be aware that the facility cannot operate the UST system prior to the EHD <br />inspection and approval. <br />4. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically, present at the work site W10 possesses all at the <br />folmdng: <br />a. One of several permissible valid, current contractors licerrsas issued bytle <br />Contractors State Ucense Board (may be issued to the installation company), or a <br />Tank Testers license Issued to the InSwidual by the State Water Board, and <br />b. A valid, current California UST Service Technician certificate issued by the <br />International Code Council (ICC), and <br />o. A valid, current certificate of training train the manufacturer(s) of the components) <br />being tested, repaired, or serviced. <br />
