" PUBLIC HOA'LTH SERVICfS <br />SAN JOAQUIN COUNTY <br />ENVIRONMENTAL HEALTH DIVISION <br />Karen Furst, M.D., M.P.H., Health Officer <br />304 East Weber Avenue, Third Floor • Stockton, CA 95202 <br />209/468-3420 <br />April 11, 2001 <br />CONDITIONS OF APPROVAL <br />Site address: Quik Stop #144, 7272 West Lane, Stockton, Ca. <br />Your application for permit to install new dispensers, dispenser containment boxes, flex <br />connectors and new monitoring system at above location has been approved with the <br />following conditions: <br />1. Contact Fire Department for permit. <br />2. All new submittals supercede the previous ones. <br />3. Automatic pump shutdown must satisfy all conditions listed in section 2636(g) of <br />California Code of Regulations (CCR). <br />4. Line leak detector (LLD) is required on all pressurized piping. Submit information to <br />San Joaquin County Environmental Health Division (EHD) for approval prior to <br />installing new LLD. <br />5. Line tightness test is required prior to placing new tank system into service. Submit <br />result to EHD. <br />6. Update monitoring and response plan. Submit a copy to EHD prior to scheduling <br />final inspection. <br />7. Submit as -built plan to EHD prior to scheduling final inspection. As -built plan must <br />be site-specific, accurate and drawn to scale. <br />8. Contact Michael Kith at 209-468-3444 for the following inspections: <br />a. Primary piping test. <br />b. Secondary piping test may be required. <br />c. 24 hours lake test. <br />d. Final inspection. <br />All appointments shall be scheduled at least 48 hours in advance. <br />9. Any changes in information provided to EHD must be submitted to EHD for approval <br />prior to commencing work. <br />10.
