An approved copy of the response plan was not available for inspection. A copy of this plan shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to check and note the correct test dates for the secondary containment test on the January 3, 2017 designated operator monthly inspection report. The designated operator inadvertently listed 9/17/13 instead of 9/29/16 for secondary containment testing. The designated operator listed 12/18/16 as the ICG designated operator certification expiration date instead of 12/4/16 on the August 1, 2016 and September 7, 2016 designated operator monthly inspection reports. The designated operator inadvertently entered March 20, 2015 as the ICC designated operator certification expiration date on the December 1, 2016 designated operator monthly inspection report. The designated operator inadvertently entered 11/13/2018 instead of 11/4/2018 for the ICC designated operator certification expiration date on the January 3, 2017 and March 1, 2017 designated operator monthly inspection report.s During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 211 CCR 2715(f) Designated operator employee training not performed or log not kept. The designated operator employee training for Rajwinder Kaur, Surinder Kaur, and Balwinder Somal were not performed within the last year. Rajwinder Kaur was last trained on 7/22/15 and Surinder Kaur and Balwinder Somal were last trained on 6/14/2016. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD.
