A copy of this plan shall be accessible on <br />site at all times. Immediately locate and retain a copy on site. <br />This is a minor violation. <br />209 <br />CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br />The designated operator failed to check and note the correct test dates for the secondary containment test on the <br />January 3, 2017 designated operator monthly inspection report. The designated operator inadvertently listed <br />9/17/13 instead of 9/29/16 for secondary containment testing. The designated operator listed 12/18/16 as the ICG <br />designated operator certification expiration date instead of 12/4/16 on the August 1, 2016 and September 7, 2016 <br />designated operator monthly inspection reports. The designated operator inadvertently entered March 20, 2015 as <br />the ICC designated operator certification expiration date on the December 1, 2016 designated operator monthly <br />inspection report. The designated operator inadvertently entered 11/13/2018 instead of 11/4/2018 for the ICC <br />designated operator certification expiration date on the January 3, 2017 and March 1, 2017 designated operator <br />monthly inspection report.s During the monthly inspection, the designated operator shall check that all required <br />testing and maintenance for the UST system have been completed and document the dates they were done. <br />Ensure that designated operators performing monthly inspections at this facility are including all of the required <br />information on the reports. <br />This is a repeat violation, Class II. <br />211 <br />CCR 2715(f) Designated operator employee training not performed or log not kept. <br />The designated operator employee training for Rajwinder Kaur, Surinder Kaur, and Balwinder Somal were not <br />performed within the last year. Rajwinder Kaur was last trained on 7/22/15 and Surinder Kaur and Balwinder Somal <br />were last trained on 6/14/2016. The designated operator shall train facility employees for which he or she is <br />responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall <br />include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4.
