Mishra <br />Subject: 2010 Notice of Annual Inspection/Certification of LLNL S300 USTs-Scheduled for 8/30/2010 <br />To: Stacy Rivera, Registered Environmental Health Specialist <br />San Joaquin County Environmental Health Department <br />Per your request this notice is being faxed to you at (209) 468-3433, and also sent to you by email. <br />The Lawrence Livermore National Laboratory, at its Site 300 Experimental Test Site in San Joaquin County, plans to perform annual <br />maintenance and service checks, and certification tests of the monitoring equipment for regulated USTs "... for operability, proper <br />operating condition, and proper calibration." as required by 23 CCR 2630 (d), 23 CCR 2637 (b), and 23 CCR 2712 (b). <br />Per your request copies of the test results will be submitted to your office by fax at (209) 468-3433 after the tests have been completed <br />and also electronically forwarded to you. The monitoring equipment service checks and tests are required to be performed by a <br />trained/experienced individual who understands the systems, is a CA Licensed Contractor (A, C-10, C-34, C-36, or C-61 (D40) per <br />H&SC 25284. 1 (a)(5)(D)(ii) and 23 CCR 2637 (b)(1)(A)), has a current ICC CA UST Service Technician certificate, and is <br />trained/certified on each component in the tank system being certified. LLNL has made arrangements to have the following vendor, <br />who has experienced and trained individual(s) that meet all the testing and training requirements, perform the certification tests on the <br />petroleum USTs at LLNLs Site 300 Experimental Test Site in accordance with the following. <br />Contractor: Thornton Environmental Construction <br />Site 300 Monday 8/30/2010 commencing at approximately 9:00 AM (Meet at the LLNL 5300 B889 Parking Lot where a Security <br />Escort will meet you.) <br />Site 300 Tuesday 8/31/2010 commencing at approximately 9:00 AM (Extra day only if needed.) <br />B882 882-DIU1 (diesel) (security escort required) 8/30/2010 <br />B879 879-D1U1 (diesel) 8/30/2010 and 8/31/2010 only if needed <br />B879 879-G3U1 (gasoline) 8/30/2010 and 8/31/2010 only if needed <br />This notice is being sent to you to confirm our previous conversations that you plan to witness the certification tests and that the <br />dates/times listed above fit your schedule and are agreeable. <br />Arrangements will be made so you may pick up your access badge at LLNLs Site 300 Experimental Test Site. <br />Arrangements will be made for your access into the restricted area at B882 where one of the USTs is located. <br />If you have any questions, please call me at (925) 423-6626 or contact me by email. <br />Regards, Bill Schwartz 8/5/2010 <br />William W.
