Maintenance and monitoring records for the last three years were not found on site. Secondary containment testing report for testing completed on 5/7/15 and the monitoring system certification (MSC) testing report for 2014 were not available during the inspection. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. A copy of the secondary containment testing report and 2014 MSC report were made available during the inspection. This was corrected on site. This is a repeat violation, Class ll. 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to document all the alarms from the attached alarm history on the July 25, 2016 designated operator monthly inspection report and failed to check that they were responded to appropriately. The missing alarms include: L8: South Piping Sump Low Liquid Alarm on July 1, 2016. The designated operator addressed the L6:"92" sump STP sump fuel alarms on the designated operator monthly inspection reports for: 7/25/16, 10/24/16, 11/22/16, 1/25/17, 2/24/17, 3/28/17 and 4/24/17 ; however, the designated operator failed to check the box"Yes" for Item 4"Has each alarm for the previous month been responded to appropriately?" Instead the designated operator checked "NA". During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. The designated operator failed to check and note the correct test dates for the secondary containment testing on the June 2016 through April 2017 designated operator monthly inspection report. Instead of entering 5/7/2015, the designated operator entered 5/7/2016. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II.
