An alarm log was not found on site for the following alarms: L6 - 92 turbine sump - fuel alarm - <br />8/9/11, 6/23/11, 4/18/11, 2/17/11, and 12/18/10; L5 - 87 master turbine sump - fuel alarm - 6/23/11; L4 - <br />87 siphon piping sump - fuel alarm - 7/23/11, 7/6/11, 6/23/11. Keep a record of the date, identification, <br />corrective action, and service technician receipt (if applicable) for each alarm. These records shall be <br />kept on site for at least 3 years. Immediately locate and maintain the alarm logs on site and submit <br />copies to the EHD by September 22, 2011. <br />12 & 16. The vapor condensate trap suction line secondary containment pipe was plugged where it <br />drains into the 87 turbine sump. The testing technician removed the plug during the inspection. <br />18. The 87 siphon and 91 spill containers failed when tested. All spill containers shall have a minimum <br />capacity of five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. <br />The new spill containers passed the capacity and tightness tests under permit (SR0063300) on the day <br />of the inspection. <br />23a. A new designated operator, Steve Zwahlen, was hired in August 2010 and notification was not <br />provided to the EHD within 30 days of the change. A new designated operator, James Flowers, was <br />hired in April 2011 and notification was not provided to the EHD within 30 days of the change. Any <br />changes shall be reported to the EHD with 30 days. Submit notification to the EHD identifying all the <br />designated operators for this facility by September 22, 2011. <br />23c. The designated operator failed to check and note the correct test dates for the secondary <br />containment test, spill container test and monitoring system certification on the 8/10 through 7/11 <br />designated operator monthly inspection reports. The correct dates are: 8-22-11 for the monitoring <br />system certification and spill container test, and 4-21-09 for the secondary containment test. During the <br />monthly inspection, the designated operator shall check that all required testing and maintenance for the <br />UST system have been completed and document the dates they were done.
