The designated operator failed to document all the alarms from the attached alarm history on the <br />12/2010 and 7/2011 designated operator monthly inspection reports and failed to check that they were <br />responded to appropriately. The missing alarms include: L6 - 91 turbine sump - fuel alarm -12/18/10, <br />6/23/11; L5 - 87 master turbine sump - fuel alarm - 6/23/11; L4 - 87 siphon piping sump - fuel alarm - <br />6/23/11, 7/6/11, 7/23/11. During the monthly inspection, the designated operator shall review the alarm <br />history for the previous month, check that each alarm was documented and responded to appropriately, <br />and attach a copy of the alarm history with documentation taken in response to any alarms to the <br />monthly report. Include a service technician receipt for each alarm (if applicable). Ensure that <br />designated operators performing monthly inspections at this facility are including all of the required I <br />C <br />information on the reports. <br />Notes <br />1. Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by September 25, 2011. <br />i 2. All future work done on the vapor condensate trap system will require an approved repair permit, <br />before work is started, from the San Joaquin County Environmental Health Department to verify <br />compliance with Title 23 California Code of Regulations and California Health & Safety Code Chapter <br />6.7. <br />f <br />3. Triennial secondary containment testing at the facility is due on 4-21-12.
