The <br /> permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution <br /> Control District for any applicable permits and requirements. <br /> 2. Contact Muniappa Naidu at(209)468-3439 to arrange the following inspection <br /> appointment(a 48-hour notice is required for inspection appointments): <br /> a. Removal/Repair and testing of the return product piping—Sampling may <br /> be required(Pressure and Soap Test). <br /> b. Removal of new piping and repair of STP sump. <br /> c. Lake test for the STP sump. <br /> d. Final secondary testing for the entire system in operating condition. <br /> 3. Since concrete is being broken near the UST system, a hydrostatic test or tank <br /> secondary containment test must be performed by a SWRCB approved tester in <br /> accordance with the manufacturer's guidelines and witnessed by EHD. This test <br /> must be performed when the UST system is in operating condition. The test <br /> results must be submitted to SJCEHD within 7 days of completing this project. <br /> 4. Any changes in information and scope of work must be submitted to EHD for <br /> approval prior to commencing work. All new addenda supersede the previous <br /> ones. <br /> 5. Individuals installing UST Systems or components shall possess or work under <br /> the direct and personal supervision of an individual physically present at the work <br /> site who possesses all of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board(may be issued to the installation <br /> company), and <br /> b. A valid, current International Code Council (ICC)UST <br /> Installer/Retrofitting certificate and <br /> c. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s)being installed. <br />
