May 2013 <br /> pa re <br /> 8,Inc. <br /> Project No. 1301-0621 9.®..LOGISTSa <br /> 13 <br /> 2.0 SCOPE OF WORK <br /> 2.1 PREPARATION OF SITE HEALTH &SAFETY PLAN <br /> Padre has prepared a site-specific HSP for the subject soil assessment program, which <br /> is attached as Appendix A. The HSP includes procedures, equipment, and materials/supplies <br /> employed to protect worker and community health and safety during the course of the <br /> assessment activities. The HSP also includes provisions for daily tailgate safety meetings. <br /> The HSP portion of the document will detail all necessary procedures, monitoring <br /> equipment, and other methods that will be employed to protect worker and community health <br /> and safety throughout the course of the assessment program. The HSP will list the chemical <br /> compounds most likely to be encountered during the field operations, and will present <br /> established exposure levels that will not be exceeded. In addition, the HSP will contain a <br /> hospital location map. All on-site personnel will be required to familiarize themselves with and <br /> sign the HSP in an attempt to minimize the potential site hazards. <br /> 2.2 PERMITS <br /> Padre will submit the appropriate drill hole permit application, fee, and a copy of this <br /> workplan to the San Joaquin County Environmental Health Department (SJCEHD) and will <br /> coordinate with SJCEHD regarding abandonment of the drill holes a minimum of five days in <br /> advance. <br /> 2.3 CLEARANCE OF ASSESSMENT LOCATIONS FOR UTILITIES AND PIPELINES <br /> Padre will complete a pre-assessment site visit approximately 5- to 10-days prior to the <br /> scheduled start of field activities. During the course of the pre-assessment site visit, Padre will <br /> demarcate the surface location of each proposed drill hole location with a painted white symbol. <br /> Also, during this site visit, any significant site features (i.e. overhead power lines, sewer lines, <br /> trees, etc.) will be noted and discussed with Padre's project manager prior to the start of work. <br /> Immediately following the site visit, USA will be notified. As part of the notification, USA will be <br /> informed of the type of work planned, including the number of drill hole locations, and will be <br /> requested to identify all underground utilities in the vicinity of the Project Site that may be in <br /> conflict with the proposed drilling locations. Additionally, Padre will utilize a private utility <br /> locating company (Subtronic Corporation of Concord, Ca) in an attempt to locate the <br /> underground fuel lines and additional underground utilities in the area of the planned <br /> assessment. <br /> 2.4 DRILL HOLE ADVANCEMENT <br /> Padre proposes to advance seven angle drill holes (DH-1 through DH-7) along the <br /> pipeline corridor to approximate vertical depths of 7 feet below surface grade which will be a <br /> minimum of 2 feet beneath the existing piping. Based on the proposed depth, a significant <br /> release from the piping will be identified.
