The EHD must witness the functionality of the monitoring system to ensure that <br />monitoring has been restored to meet the requirements of California Code of <br />Regulations Title 23 and the Health & Safety Code. <br />7. If concrete is broken near the UST, a tank secondary containment test must be performed <br />by a SWRCB licensed tank tester in accordance with the manufacturer's guidelines. <br />Results must be received by the EHD within seven days of returning the facility to <br />operating condition. <br />8. If concrete is broken near the product lines, a line secondary containment test must be <br />performed by a SWRCB licensed tank tester in accordance with the manufacturer's <br />guidelines. Results must be received by the EHD within seven days of returning the <br />facility to operating condition. <br />9. If electrical penetration is added to UDC or sump, a 24 hour lake test or its equivalent <br />will be required to be witnessed by this office. <br />10. An "Underground Storage Tank Certification of Installation/Modification" form (form C) <br />shall be submitted by the owner or the owners' agent [pursuant to (CCR) 2635(d)] before <br />this upgrade will be considered complete by EHD. <br />11. Permit expires 180 days from date of approval.
