Financial responsibility documents have not been submitted to the EHD. Current financial <br /> responsibility documents are required to be submitted annually. The copy on file is dated July 23, 2012 <br /> and April 25, 2012. Received an updated copy dated July 2013, during the inspection. Take all <br /> necessary precautions to complete and submit a current copy of the financial responsibility annually. <br /> 201. The L6 87 annular sensor alarmed on July 13 and 15, 2013. The L1 91 turbine sump sensor <br /> alarmed on December 20, 2012 and on March 30, 2013. The designated operator did not document <br /> these alarms on the monthly designated operator report. The facility did not document the July 13, 2013, <br /> L6 87 annular sensor alarm, the March 30, 2013, L1 91 turbine sump sensor alarm, nor the December <br /> 20, 2012, L1 91 turbine sump alarm. Alarm maintenance records for these alarms were not on site. <br /> These records shall be kept on site for at least 3 years. Immediately locate and maintain the facility <br /> alarm log and alarm maintenance records on site and submit copies to the EHD by August 25, 2013. <br /> 204. The designated operator failed to document all the alarms from the attached alarm history on the <br /> July, April, and January 2013 designated operator monthly inspection reports and failed to check that <br /> they were responded to appropriately. The missing alarm includes, the L6 87 annular sensor that <br /> alarmed on July 13 and 15, 2013, the L1 91 turbine sump sensor that alarmed on December 20, 2012, <br /> and on March 30, 2013. The January 2013 designated operator report lacked the L1 91 turbine sump <br /> sensor alarm history print out. During the monthly inspection, the designated operator shall review the <br /> alarm history for the previous month, check that each alarm was documented and responded to <br /> appropriately, and attach a copy of the alarm history with documentation taken in response to any <br /> alarms to the monthly report.
