Victor Rd. Lodi, CA 95240 Program: UST <br /> SUMMARY OF VIOLATIONS <br /> CLASS 1,CLASS II,or MINOR-Notice to Comply) <br /> 206. The designated operator employee training for Muhammad Ali was not documented for this facility <br /> and he was left to work alone at this facility during the inspection. The designated operator shall train <br /> facility employees for which he or she is responsible in the proper operation and maintenance of the <br /> UST system once every 12 months. The training shall include, but is not limited to.. <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and <br /> submit a copy of the training records to the EHD by August 25, 2013. <br /> 310. Liquid was observed in the 91 turbine sump. If liquid could enter into the secondary containment by <br /> precipitation or infiltration, it must be removed and disposed of properly. Take the necessary precautions <br /> to ensure that the secondary containment of the UST system is maintained free of liquid. During the <br /> inspection the technician removed the liquid from this sump and placed it into the site hazardous waste <br /> drum. Continue to ensure that any liquid from the secondary containment sumps are managed properly <br /> per California Title 22 Hazardous Waste Regulations. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by August 25, 2013. <br /> Ensure that all required information has been entered in the online California Electronic Reporting <br /> System. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO R INSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> Aris Cacapit (209) 468-3453 <br /> EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
