San Joaquin County RECEIVED <br /> Envionmental Health Department APR102015 <br /> 1868 East Hazleton Aveue <br /> Stockton, Ca 95205 Vl ir'/SES I HEALTH <br /> 8 <br /> Dear Michelle Henry, <br /> We have made the following corrections to correct all the violations during our <br /> annual monitoring test. <br /> Item#100:I have contacted Epic compliance to send us a hard copy of the Secondary <br /> Containment Test so I can upload it on the CERS. <br /> Item#201, 207: I contacted our UST operator Alex and he said the alarm was trigger <br /> because the sensor malfunctioned which it triggered the alarm but then went away <br /> on its own. Alex also spoke to STACY RIVERA and was told that he must document <br /> any type of malfunctions and also contact a service technician if there are any types <br /> of malfunctioning occurring on site. <br /> Item#204: I printed a current copy of the monitoring plan that was on my computer <br /> and display it so all employees will have access to it. <br /> Item#306: The sensor was reposition by Epic Service technician to the lowest point <br /> so it can detect a leak at the earliest possible opportunity. <br /> Item#319: The water in the secondary containment was due to the secondary <br /> containment test on October 14, 2014. The technician did not remove all the water <br /> after the test. I ended up asking the technician to remove all the liquids from the <br /> sump and disposed the water in the metal hazardous drum. <br /> Item 323: Service technician adjusted the leak detector and retested it for it to pass. <br /> Item 325: I immediately contacted Elite IV contractors to fix the spill container. <br /> Elite IV applied for the permit to get it done and the spill container was fixed a <br /> couple of days. The Spill container was tested and passed before we took a fuel <br /> delivery <br /> Those are all the procedures we have taken to correct our violations.
