Maintenance & monitoring records not maintained for 3 years. Unable to locate the most recent <br />secondary containment test results performed on 12/16/2011 in the facility's binder on site. Binder <br />contained secondary containment test results for 2008 and 2005. These records shall be kept on site for <br />at least 3 years . Immediately locate the secondary containment test results and maintain the record on <br />site. Explained how this was completed. <br />301,303,304,305. UST system is not operational to prevent unauthorized releases, including spills and <br />overfills. UST monitoring system is not maintained to detect a leak at the earliest opportunity. Monitoring <br />system is not present in the interstitial space of the double wall UST. There is no continuous monitoring <br />system for the underground piping and under dispenser containment that activates audible and visual <br />alarm or stops the flow of product at dispenser when it detects a leak. <br />The Incon TS-1000EF1 motherboard is inoperable and unresponsive. The technician was on the phone <br />with an Incon representative trouble shooting the monitoring system. It appears there are no voltage <br />reading, thus the sensors are unable to send messages to the Incon. Incon no longer produces this <br />monitoring system and there are no replacement parts. The underground storage tank system shall be <br />operated to prevent unauthorized releases. <br />- An approved permit SR0065874 dated 10/15/12 was taken out to replace the existing Incon system & <br />sensors with a Veeder-Root monitoring system & sensors. The work is planned in the next few days. <br />Fuel station is to remain closed & no diesel deposition is allowed into the tank until monitoring <br />system is repaired and tested under inspection of inspector. Provide copy, for file and inspector, of the <br />new Monitoring Plan once system is replaced. <br />- Caution tape is placed around entire dispenser. All master dispensers are chained and locked. Satelite <br />dispenser cannot dispense fuel. Turbine breaker is turned off. <br />- Current Inventory diesel: Gross 11357.5 gal. Net 11288 gal. Product level 65.467 inches. Ullage <br />8804.9 gal <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />Inspector..
