0 6 <br /> c. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s) being tested, repaired, or serviced. <br /> And in addition <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 4. Any changes shall be submitted to SJC Environmental Health Department(EHD) <br /> for approval prior to commencing work. All new submittals supersede the <br /> previous ones. <br /> 5. The following information must be submitted within 30 days of the sample date: <br /> a. Analytical results sent directly from a California certified laboratory or <br /> this office WILL NOT accept them. <br /> 6. No gasoline, diesel, or other hazardous material or waste can enter the <br /> sewer/septic or storm water system. If it does it will be considered illegal <br /> hazardous waste disposal and enforcement actions could follow. <br /> 7. If any fuel is to remain in the underground storage tank, it must be continuously <br /> monitored. <br /> 8. All installation and testing shall be in compliance with manufacturer's <br /> instructions, Section 2635 and 2636 of CCR. <br /> 9. A"Certificate of Compliance for Underground Storage Tank Installation Form C" <br /> shall be submitted by the owner or the owner's agent before this installation will <br /> be considered complete by EHD. <br /> 10. Secondary containment testing must be performed on all product lines exposed <br /> during work. Test results must be received by this office within 7 days of <br /> completion of work. <br /> 11.
