Since concrete will be broken near the USTs and/or piping, a secondary containment <br /> test must be performed on the USTs and/or piping once concrete is poured in <br /> accordance with the manufacturers guidelines. <br /> 5. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br /> b. A valid, current California LIST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s)of the component(s) <br /> being tested, repaired, or serviced. <br /> 6. Qualified installers must possess or work under the direct and personal supervision of an <br /> individual physically present at the work site who possesses all of the following: <br /> a. A valid, current contractor's license issued by the Contractors State License Board, <br /> and <br /> b. A valid, current certificate of training from the manufacturer(s) of the component(s) <br /> being installed, and <br /> c. A valid, current UST Installation/Retrofitting certificate issued by the International <br /> Code Council (ICC). <br /> 7. Update and submit a copy of the 'as built' plans to the EHD upon completion of work. <br /> 8. A"UST Certification of Installation/Modification"form shall be submitted by the owner or <br /> the owner's agent(CCR Section 2635(4)) before this upgrade is considered complete by <br /> EHD. <br /> 9. All components must be certified by an independent testing organization and labeled as <br /> required by the organization (UL, ULC, ANSI, API, ASTM, etc.). See California Code of <br /> Regulations (CCR)Title 23, Chapter 16, Table B for a complete list. <br /> 10. Permit expires 180 days from the date of approval.
