Yosemite Avenue, Manteca <br />San Joaquin County Public Health Services Environmental Health Division (EHD) has <br />performed a preliminary review of your submittal for application to install <br />underground storage tanks at the above referenced site. <br />Please submit the following documentation prior to issuance of a permit: <br />1) The following changes must be made to the permit application, and <br />subm ted in triplicate: <br />Page 4 — The owner must sign at the bottom. <br />Page 8, Number 25 — The owner/operator must sign. <br />Page 5, Number 12 — The tank manufacturer, composition and model <br />must be specified. <br />V. Page 5, Number 13B — Fiberglass tanks will require an air test and <br />soap bubble test prior to installation, unless the manufacturers written <br />installation instructions prohibit it. <br />-----3>E. Page 6, Number 14A — Visual monitoring is not an acceptable <br />monitoring practice for new UST installations. <br />Page 7, Number 16 — Bouyancy calculations (stamped by a <br />Professional Engineer) must be submitted for this tank installation. <br />s/ Page 8, Number 23 — The total number of tanks on site after this <br />installation will be three. (Compartmentalized tanks are counted as <br />separate tanks) . <br />2) Please submit a copy of the tank manufacturers written installation <br />instructions with an installation checklist. <br />3) The following changes must be made to the plans, and submitted in triplicate: <br />. A cross section of the compartmentalized tank is required. <br />Sheet TL3 number 7 must clearly show double-wall piping in the <br />piping trench. <br />The location of the monitoring system panel needs to be shown on the <br />plans. <br />A Division of San Joaquin County Health Care Services <br />
