13.4 RESPONDING TO EMERGENCIES <br /> In the event an actual or suspected incident where personal injury or <br /> illness occurs,the FSO should take the following actions sequentially as <br /> listed: <br /> • Assess scene; <br /> • Don appropriate PPE; <br /> • Remove the exposed or injured person(s) from immediate danger if it <br /> is safe to do so; <br /> ® Decontaminate affected personnel as appropriate; <br /> • Drive injured person or obtain ambulance transport to the local <br /> hospital in the event of any injury or illness deemed to require medical <br /> surveillance or treatment; and <br /> • Evacuate other personnel until it is safe for work to resume. <br /> 13.5 REPORTING EMERGENCIES <br /> At the earliest time practicable following the occurrence of the emergency <br /> situation, the FSO will contact the PM and HSC to advise them of the <br /> situation. The PM will then be responsible for promptly informing the <br /> following parties about the emergency: <br /> • Injured/involved personnel's supervisor; <br /> • Partner-In-Charge; and <br /> • Client Contact. <br /> A flow chart outlining the communication process is included in <br /> Appendix J. <br /> The HSC will be responsible for informing the Division H&S Advisor and <br /> the Sacramento Office Manager. In the case of an Incident, the HSC will <br /> promptly begin formal documentation of and investigation into the root <br /> causes of the Incident following the occurrence of the Incident.
