14.0 REPORTING OF SAFETY EVENTS
Safety events are occurrences or conditions that may contribute to or
result in an injury, occupational illness,or property damage. ERM seeks
to learn from the investigation of the following types of safety events:
• Remarkable safe behaviors;
• Unsafe acts and unsafe conditions;
• Near misses; and
• Incidents (injury,illness,property damage,fire,or chemical spill).
14.1 REMARKABLE
SAFE BEHAVIORS, UNSAFE ACTS, UNSAFE
CONDITIONS,AND NEAR MISSES
As stated in Section 5, each day all ERM employees and subcontractors
must fill out a PSC prior to starting work and keep it with them
throughout the day. Should a safety event(remarkable safe behavior,
unsafe act,unsafe condition,or near miss) occur,the employee should
record the event on their PSC and then indicate the steps taken towards
resolution. The FSO will review the PSCs weekly (or more often for large-
scale construction activities) and will take the following steps:
• Enter the safety event into ERM's Event Communication System(ECS),
which tracks all safety events and resolutions company-wide and
automatically notifies senior project personnel and individuals within
the company;
• Ensure that an appropriate corrective action was taken to mitigate the
safety event and prevent a future reoccurrence;
• Complete the necessary paperwork; and
• Notify all field personnel of the safety event so that individuals
involved in the project may learn from the actions of their peers.
14.2 INCIDENTS
ERM defines an Incident as a safety event that results in an individual or
object becoming hurt, sick, or broken(including fire,spills, and fatalities).
Anytime that an Incident occurs on site, the following steps must be taken:
• Work must be stopped immediately;
ERM 14-1 DEERE/129740-9/29/2011
