Site-specific hazards related to <br /> this task have been incorporated by the project team. The JHA should be reviewed regularly with site personnel <br /> who will be performing this task. <br /> Task Description: <br /> Guidelines for loading trucks with heavy equipment e. .loader or excavator). <br /> Hazard Analysis: <br /> Task Step Hazard Control Measures <br /> Off-site preparation. Untrained workers operating heavy equipment ERM policy and practice is that our employees do not operate <br /> pose potential life-threatening hazards to heavy equipment except in unusual circumstances. Only <br /> employees. employees with proper training and experience are authorized <br /> to operate heavy equipment. <br /> Subcontractor personnel operating heavy equipment must be <br /> trained and/or have demonstrated experience operating such <br /> equipment. ERM must be in possession of evidence of <br /> training and/or experience prior to Subcontractor personnel <br /> operating such equipment. <br /> All heavy equipment must meet applicable design standards <br /> (ANSI,etc.).
