Injury to operator and those in immediate vicinity. Before starting operations,operators must ensure no one is <br /> working on or near machinery. If equipment is to be operated <br /> in proximity to other workers,a spotter must be working in <br /> tandem with the operator. <br /> All heavy equipment must be inspected daily to ensure good <br /> working order. Critical safety items,such as brakes,backup <br /> alarms,horns,etc.,must be in working order. Machinery with <br /> critical safety items in disrepair may not be used until they are <br /> fixed. Rental equipment will be thoroughly inspected before <br /> first use and after last use to ensure that it is in proper <br /> functioning order. <br /> Operators must operate equipment while wearing seatbelts,if <br /> provided,and at reasonable speeds. Mounting/dismounting <br /> a moving machine is prohibited. Do not transport personnel <br /> or equipment in machinery not designed for this purpose. <br /> Overhead obstructions must be assessed before operating <br /> machinery. If equipment is to be operated in proximity to <br /> overhead obstructions,a spotter must be working in tandem <br /> with the operator. Safe working distances must be specified <br /> in the HASP or JHA supplied by the subcontractor. <br /> ERM North America 2 Form Rev.:05-03 <br />
