North America Job Hazard Analysis <br /> Truck Loading <br /> ERM <br /> Project Name: John Deere Landscapes(Former LESCO Facility) <br /> Project Number: 0129740 <br /> Job/Task Name: Truck Loading <br /> JHA No.: 15 <br /> Task Step Hazard Control Measures <br /> Loading soil or railroad ties Injury/damage to individuals/vehicles in the area A clearly defined work zone must be established that <br /> into end-dump trucks. from heavy equipment,dump trucks,or debris. identifies the loading area and directs trucks to the proper <br /> loading location. This work zone will isolate the loading area <br /> and keep it free from other non-essential site workers and <br /> vehicles. Regular vehicle traffic will be directed away from <br /> the loading area with cones or signs. Trucks will be staged <br /> out of the way,so as not to pose a hazard to regular site <br /> traffic. <br /> The loading zone must be clear of all hazards. Any hazard <br /> that can not be removed will be clearly marked with caution <br /> tape or orange marking paint. If working in tight quarters,a <br /> spotter will be used. <br /> Workers in the area may not approach from the far side(non- <br /> loading side)of the dump truck without first notifying the <br /> loader/excavator operator. This is to prevent injury from <br /> objects that may fall over the rail of the truck while the truck is <br /> being loaded. <br /> If the truck driver wishes to observe the loading process,they <br /> will be instructed to remain at a safe distance while loading is <br /> occurring. Truck drivers must remain outside of the swing <br /> radius and/or travel path of the heavy equipment. The <br /> loader/excavator operator must be aware of the truck driver's <br /> location at all times. Level D PPE must be worn by all site <br /> workers,including truck drivers,present in the work zone. <br /> Ending heavy-equipment Leaving equipment in a non-neutral position poses All heavy equipment must be placed in a neutral position <br /> operations. contact hazards. when not in operation. Dump truck beds must be lowered, <br /> buckets must be at ground level,etc. Keys must be removed <br /> from all heavy equipment when not in use. <br /> ERM North America 3 Form Rev.:05-08 <br />
