y %no, Chaney,Walton&MCCA (LLC) <br /> Chronology of UST Operation by VFO <br /> According to PHS/EHD records, integrity tank tests were performed annually as required. <br /> The pre-acquisition testing results indicated that the results were within the NFPA 329 <br /> Guidelines, and that the UST system was considered "tight". <br /> The following is a brief summary of the available PHS/EHD records, which relate <br /> directly to the UST removal in November 1991 and its former operation: <br /> • Laboratory analysis results of collected soil samples were issued on December 4, <br /> 1991 from a California certified testing facility for distribution to VFO and <br /> PHS/EHD. <br /> • The Underground Tank Disposition Tracking Record indicated that the tank was <br /> received for final destruction on November 25, 1991. <br /> • Regarding the backfilling practices, according to Mr. Thorpe of Jim Thorpe Oil, Inc., <br /> the UST removal contractor,their typical procedure was to backfill a portion of the <br /> tank excavation with clean pea-gravel, and to emplace a portion of the stockpiled soili� <br /> on top of the pea-gravel to grade. He noted that there was nothing atypical about the <br /> Site UST removal, nor did he recall any indication of a release at the Site. <br /> �E , <br /> • The UST was removed on November 12, 1991. The removal was witnessed by a <br /> PHS/EHD representative, Dole Fresh Fruit Company representatives, a laboratory <br /> sampling technician, and the tank removal contractor. Two soil samples were <br /> collected, which was the minimum number of samples for a UST of that particular <br /> size. One sample was collected from the base of the excavation, and a second sample <br /> was collected from the stockpile of excavated soil. No unusual remarks or indications/ <br /> of a possible gasoline petroleum hydrocarbon release were found in the PHS/EHD t?t°� <br /> records. <br /> • An Application for UST Closure Permit is submitted to PHS/EHD by VFO on <br /> October 21, 1991. The Closure Plan is approved by San Joaquin County staff on <br /> October 31, 1991. <br /> • Integrity tank testing for VFO was performed on October 25, 1990. The UST system <br /> passed and was tight. <br /> • Integrity tank testing for VFO was performed on October 18, 1989. The UST system <br /> passed and was tight. <br /> • Integrity tank testing for VFO was performed on October 31, 1988. The UST system <br /> passed and was tight. <br /> • Integrity tank testing for VFO was performed on October 28, 1987. The UST system <br /> passed and was tight. <br /> • First inspection by PHS/EHD for VFO was December 5, 1985 to confirm the required <br /> monitoring alternatives for the UST system. <br /> Included within the files for the former UST were the PHS/EHD Worksheets for Site <br /> Closure Evaluation, dated November 8, 1991: It appears that the Worksheets were <br /> completed following the receipt of laboratory analysis results.
