M and M Automotive V N001 November 1994 <br /> Site Specific Safety Plan Soil Boring/Sampling <br /> WES Project No. 4070-T Well Installation <br /> 13. 1 EXAMINATION REQUIREMENTS <br /> All WES personnel on-site shall have successfully completed a pre-placement or <br /> periodic medical examination in accordance with established WES policies and <br /> procedures, and consistent with the provisions of the OSHA carcinogen standards. <br /> This examination shall include a complete medical and occupational history, <br /> physical examination, and selected biological sampling. Laboratory studies <br /> include a complete blood count (CBC), urinalysis, chemistry panel (SMAC), <br /> pulmonary function (FEV and FVC), chest X-ray, audiometer, and vision <br /> screening. Additional testing will be conducted as deemed appropriate by the <br /> occupational physician. Ongoing medical consultation and post-project testing <br /> will be provided. <br /> 13.2 SUBCONTRACTOR MEDICAL SURVEILLANCE <br /> Medical surveillance for subcontractor personnel shall be consistent with the <br /> requirements of Section 13.1 above. <br /> 13.3 EMERGENCY MEDICAL TREATMENT <br /> Emergency facilities and contact agencies (which include local hospital facilities, <br /> state highway patrol, local fire/paramedics, and local law enforcement agencies) <br /> will be established, and arrangements made with, during the mobilization phase <br /> of this project. These facilities and agencies will be contacted in the event of an <br /> employee injury or illness requiring emergency medical care. <br /> SECTION 14 <br /> DOCUMENTATION <br /> 14. 1 GENERAL <br /> Record keeping shall be consistent with OSHA regulations in all respects. The <br /> following permanent records will be maintained in the WES offices: <br /> • Safety Inspection Reports <br /> • Personnel Exposure Monitoring Records <br /> • Accident reports consistent with the established WES procedures <br /> 14.2 MEDICAL RECORDS <br /> Permanent medical records shall be maintained in confidential files by the <br /> contract physician/medical clinic. <br /> 14.3 TAILGATE SAFETY MEETINGS <br /> Short job site Tailgate Safety Meetings will be conducted at the beginning of each <br /> shift for each job or more frequently if new employees arrive at the job site. All <br /> workers and supervisors will be expected to attend. <br />
