Environmental Health Department <br />AMENDED <br />4/8/2020 <br />RCRA Large Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />March 10, 2020 <br /> Facility Address: <br /> 1777 W YOSEMITE AVE, MANTECA <br /> Facility Name: <br /> KAISER FOUNDATION - MANTECA <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br />OBSERVATION: Hazardous waste containers were observed on-site with incomplete labeling. The containers <br />were: <br />- Located in the hazardous waste main storage area: <br />1. A black container storing methylene blue, sodium salicylate, methanol, and serum controls, was missing the <br />hazardous properties, and was marked with two accumulation start dates of 3/18/19 and 7/18/18 (see photos IMG <br />1150-1). <br />2. A black container with a white lid was missing the composition (contents) (IMG 1152). <br />3. A black container with a white lid was missing the composition (contents) (IMG 1153). <br />4. A black container with a white lid was missing the hazardous properties, physical state, and composition <br />(contents) (IMG 1154-5). <br />5. A black container with a white lid was missing the composition (contents) (IMG 1159). <br />- Located in the 1st Floor ED Overflow area: <br />6. A black container was labeled with the incorrect generator name and address, and was missing the hazardous <br />properties, physical state, composition (contents), and an accumulations start date (IMG 1163-4). The container <br />was marked with 1/14/20 as the date the container was placed in the central storage area, although it was not in <br />the central storage area. Tonia Shaw, Safety Operations Leader, stated an employee mistakingly marked the initial <br />start date in the incorrect section on the label, and should have marked the "accumulation start date" section. <br />7. A black container with a white lid was missing the hazardous properties, physical state, composition (contents), <br />and accumulation start date (IMG 1165-7). <br />8. A black container with a white lid storing silver nitrate stick waste was missing the hazardous properties, physical <br />state, accumulation start date, and was incorrectly marked as containing aerosols (IMG 1168-70) <br />- Located in the 3rd Floor Med Surg area: <br />9. Two black containers with white lids were missing the hazardous properties, physical state, and composition <br />(contents) (IMG 1173-6). <br />10. A black container with a white lid was missing the composition (contents) (IMG 1177-9). <br />- Located in the OR Holding area: <br />11. A black container with a white lid storing what appeared to be used gloves, tubing, and other solid debris, was <br />labeled as containing Gemcitabine, and was missing the physical state (IMG 1180-3). <br />12. A black container with a white lid storing "urology and lop tubing" was missing the hazardous properties (IMG <br />1184-5). The container was marked with an accumulation start date of 1/13/2020 and a date placed in the central <br />storage area of 3/6/2020, although it was not in the central storage area. <br />13. A black container with a white lid storing alcohol prostate wipe packets, was missing the physical state (IMG <br />1186). <br />- Located in the Pathology Lab: <br />14. A black container with a white lid was missing the composition (contents) (IMG 1187-8) <br />- Located in the Pharmacy: <br />15.
