Palmer, PG, CEG, HG, Consulting Hydrogeologist <br /> APPROVED: [X] <br /> ADEQUATE AND NECESSARY ELEMENTS OF THE WORK PLAN [H&S 25296.10 (c)(3)]: <br /> 1) To destroy a total of (4) four groundwater monitoring wells: MW-1, MW-2, MW-3 and <br /> MW-4. <br /> 2) Wells MW-1, MW-2 and MW-3 shall be over-drilled as indicated in the submitted well <br /> destruction work-plan. MW-4 can be pressure grouted as indicated per specifications of <br /> the well destruction work-plan. <br /> 3) All investigation derived waste, including soil and groundwater from the well destruction <br /> activities shall be stored and disposed of as stated in the well destruction work-plan. <br /> Manifests are required for removal, transportation and disposal of all wastes associated <br /> with the work. <br /> COMMENTS AND CONDITIONS: <br /> 1) The Environmental Health Department (EHD) requires one well destruction permit <br /> application @ $375.00 to be submitted. <br /> 2) A Master File Record must be completed and submitted with the EHD permits. <br /> 3) A Well Destruction Report shall be submitted within 90 days of the completion of the <br /> well destruction activities including the removal of the waste materials and all manifests <br /> associated with the removal. <br />
