Describe the project, construction techniques, and anticipated start and completion dates.
2. Define anticipated dewatering points and typical dewatering well construction.
Dewatering Operations Information
For each discrete dewatering disposal site, provide the following:
I 3. Property owner and contact information per Section 1.D of Form 200.
4. A copy of the landowner agreement authorizing discharges on the property.
5. One or more maps that show topography, property lines, buildings, surface water drainage
I courses, and proposed discharge areas.
6. The estimated daily peak and daily average volume of water to be discharged. Describe
any seasonal variation in discharge rates.
7. For each pond and other land disposal area, provide the following information:
a. Identification (name) and function of the pond (e.g., temporary holding or
percolation/evaporation);
i
b. Surface area, depth, and volumetric capacity at two feet of freeboard;
c. Height (relative to surrounding grade), crest width, interior slope, and exterior slope of
each containment berm;
d. Materials used to construct berms;
{ e. Estimated steady state percolation rate;
f. Depth to shallow groundwater below the base of the pond;
g. Overfilling/overflow prevention features; and
h. Operation and maintenance procedures.
{
