IX The DO failed to mark Yes, No, or N/A to the question"Are all sensors in under-dispenser containment located to detect a leak at the earliest opportunity?" -Section X The DO failed to mark NO under the question"Has the overfill prevention equipment inspection been completed within the past 36 months?"The overfill prevention equipment inspection has not been completed. The DO failed to mar YES, NO or N/A under the question"Has the line tightness testing been completed within the required timeframes?"and"Has the tank tightness testing been completed within the required timeframes?" -Section XI The DO failed to mark Yes or No to the question"Have all individuals performing facility employee duties received the required facility employee training within the past 12 months?" During the designated operator inspection,the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing inspections at least once every 30 days at this facility and are including all of the required information on the reports. This is a repeat violation, Class II. 211 CCR 2715(c)(2) No employee present that were trained in the operation and maintenance of the UST system by a DO. The designated operator employee training for Asim Ahmed was not performed. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s)for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. The designated operator was on site and was able to perform the training while the inspector was on site to correct this violation. This was corrected on site. This is a repeat violation, Class ll.
