Alaniz, John [EHD]
From: Chelsea Bossenbroek <cbossenbroek@northgateholdings.com>
Sent: Thursday, July 16, 2020 8:45 AM
To: Alaniz, John [EHD]
Cc: Dominique Maingot
Subject: Re: HW inspection 14900 W HWY 12
Attachments: 070720 Manifest 007440624SKS.pdf; 032520 Manifest 007439819SKS.pdf; 040220
Manifest 007314685SKS.pdf; 031820 Manifest 014788830FLE.pdf; IMG_7690.JPG; IMG_
7684.) PG
CAUTION: This email is originated from outside of the organization. Do not click links or open attachments unless you recognize the
sender and know the content is safe.
Hi John,
Please see below responses to your additional questions:
Violation 102: A determination needs to be made for the black and white 55 gallon drums. Why
wouldn't Safety-Kleen sample them?
We reached out to Safety-Kleen to inquire about them sampling the charred wood from the fire. They said they
couldn't test us and advised us to throw it away. For obvious reasons, we didn't want to do that. I reached out to
the contractor who did the demolition of the building following the fire last year and they said those containers
were not theirs and they didn't know where they came from. The prior general manager recalls that when the
fire happened, employees swept up the charred lumber from the fire and placed it in those containers. I've
attached photos of the contents of those containers. I'm not sure how we should proceed from here.
Violation 114: I need TSDF signed copies of manifests 014788835FLE and 007439819SKS
- Are you able to clarify where the 014788835FLE number for the manifest came from? It doesn't match
anything in our files; Clean Harbors doesn't have a copy either. We are wondering if you
meant 014788830FLE? That one is on file in Dropbox and appears to be complete. I've also attached a copy of
the manifest ending in 30 here.
- The fully signed version of the 007439819SKS is attached here and uploaded in Dropbox.
Violation 502: The tank assessment was incomplete. Several items such as the remaining service life of
the tank secondary containment calculations were not seen on the assessment.
Based on this comment below, please see the following italicized statement:
If the facility has had all of the waste hauled off site and is now a small generator, then I will be able to close
the tank assessment violation without any further action required. I need a statement saying the facility is now a
small generator and will continue to function as one.
