This is in accordance with Section 25297.15 of the California Health and <br /> Safety Code. Submit a complete list of record owners of fee title and their mailing addresses <br /> to the EHD. A form has been enclosed for this purpose. Please submit the information at <br /> your earliest convenience. <br /> The EHD is in need of information to determine whether the well installation permit issued for <br /> installing three monitoring wells in April 1990 was instead utilized for installing the three <br /> domestic wells currently at this site. Please provide the well construction logs for the three <br /> domestic wells associated with this case. <br /> Survey data associated with installing monitoring wells MW-5 through MW-8, and <br /> decommissioned monitoring wells MW-4, have not been uploaded to GeoTracker. Please do <br /> so immediately. Also, it appears that the top-of-casing elevations for MW-4 and MW-5 have <br /> been reversed. Please review the data and, if appropriate, revise the groundwater elevation <br /> tables and most current groundwater elevation contour map for 24 September 2013. <br /> While reviewing data associated with case closure, the EHD established that instead of seven <br /> underground storage tanks (USTs), there were eight USTs. Of these, the EHD has no <br /> documentation that a second 6,500-gallon asphalt has been removed. The tank number <br /> associated with the UST is 1798002. Please provide the current status of this UST. <br /> The EHD has reviewed historical soil and groundwater analytical data and based on this <br /> information, has determined that MW-5 must be over-drilled and MW-6 through MW-8 may be <br /> pressure-grouted. Please submit a scope of work proposal that details the conditions and <br /> construction of the wells and the method and materials to be used to destroy them. To be <br /> issued a well destruction permit, the proposal must follow the San Joaquin County Well <br /> Standards requirements. <br /> After the EHD has approved the well destruction scope of work proposal and the public <br /> participation comments have been properly addressed, you may submit an EHD Well and <br /> Boring Permit Application form along with a Licensed Contractors' Declaration (LCD), Master <br /> File Record (MFR) form, and appropriate fees for each parcel. The permit fee for the <br /> destruction of each well is $130. If it can be established that monitoring wells MW-1, MW-2 <br /> and MW-3 are really DW-1, DW-2 and DW-3, then a fee of $50.00 will be required for <br /> destroying the four monitoring wells (MW-5 through MW-8).
