The following is an itemized list of underground storage tank violations that have not been addressed for MORADA CHEVRON FAST N EASY#60* as of October 07, 2020. Open violations from April 11, 2019 inspection ❑ This violation was corrected ❑This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#303-Liquid does not flow into the sump in the event of a leak(i.e.,failure to remove test boot).. The plug under the clamshell on the 91 south STP sump product piping was not removed. The schrader valve was open, but was pointed up. Any potential release from the primary containment into the secondary containment in the 91 south piping does not flow into a collection sump. All secondary containment systems shall be sloped so that all releases will flow to a collection sump located at the low point of the underground piping. Immediately contact a properly licensed,trained, and certified contractor to ensure that any releases in the 91 south piping flows into the91 south piping sump. ❑ This violation was corrected ❑This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#318-No spill bucket,failed to contain 5 gallons, no means to keep it empty,or failed to be tested. The 91 north spill container failed to have a minimum capacity of five gallons. All spill containers shall have a minimum capacity of five gallons. Immediately contact a properly licensed, trained, and certified contractor to repair or replace the spill container under permit and inspection of the EHD. The 87 spill container failed when tested. All spill containers shall have a minimum capacity of five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. Immediately discontinue deposition of petroleum into this tank until the component is repaired or replaced by a properly licensed, trained, and certified contractor under permit and inspection of the EHD.
