The well <br /> currently on site must be sampled and the results submitted to this office on a quarterly basis. <br /> Effective immediately, please begin submitting quarterly reports to PHS/EHD. <br /> Additionally, at least two more monitoring wells must be installed at the site, with one of the wells <br /> located between the former tank pit area and the municipal well. These wells are needed to help <br /> determine the full extent of the groundwater contamination and to indicate actual site specific <br /> groundwater flow patterns. Therefore, within 30 days from the date on this letter, please submit <br /> to PHS/EHD a workplan to install the two required additional monitoring wells as discussed <br /> above. <br /> The deadlines established above are to ensure the investigation continues to progress at this site.
