PHS/EHD has the following comments for your consideration.
PHS/EHD indicated in correspondence dated July 27, 1994 that PHS/EHD staff person observed the casing of
MW1 during a site inspection that took place on May 3, 1994. The recent correspondence from RESNA
suggested that the well was not visible on April 28, 1994 when 54,076 gallons of groundwater were extracted
and therefore, could not be visible on May 3, 1994. You should. be aware that PHS/EHD conducted an
inspection on September 26, 1994 and again noted that the casi of MW1 was visible. In fact, PHS/EHD staff
brought the observation to the attention of the RENSA techniciai on site, so that there would not be any further
question regarding the observation.
Prior to issuance of the well destruction permit for which has been applied, please submit a work plan describing
the methodology to be used during destruction. Also, PHS/EHD requested that a work plan be submitted by
September 1, 1994 for a MW1 replacement well. This work plan should be submitted within 30 days from the '
date of this letter. Should you wish to schedule a site meeting to discuss the destruction methodology, please do
not hesitate to contact me at (209) 468-0337.
Donna Heran, REHS, Director '
Environmental Health Division
Mary Meays, Senior REHS Diane M.
