Patrick Hughes PERMIT/SERVICES <br /> American Moulding&Millwork Co. <br /> 2801 North West Lane <br /> Stockton, CA 95215 <br /> APPROVAL OF SITE CONTAMINATION WORK PLAN ADDENDUM, AMERICAN <br /> MOULDING AND MILLWORK, STOCKTON, SAN JOAQUIN COU TY <br /> We have reviewed the 7 May 2002 Site Contamination Work Plan Addendum.(Addendum) submitted by <br /> Advanced GeoEnvironmental, Inc. on behalf of American Moulding and Mi lwork Company(AMM) at <br /> 2801 North West Lane in Stockton (site). The Addendum proposes eight borings to collect soil and grab <br /> groundwater samples from locations of potential pollution. Along with the borings, AMM proposes to <br /> collect up to 20 soil samples from soil stockpiled on site. AMM will use the results of this investigation <br /> to prepare a health risk assessment (HRA). <br /> Section 8.4 of the Addendum states soil samples will be collected from thre borings advanced within <br /> the former teepee structure incinerator pit. Section 3.4.1 states AMM will advance one boring to collect <br /> a grab groundwater sample from the former teepee structure incinerator pit. Based on the figures and <br /> summary sections of the Addendum, it appears that AMM does intend to advance three borings in the <br /> former teepee structure incinerator pit, but it is not clear whether AMM will collect grab groundwater <br /> samples. AMM must collect at least one grab groundwater sample from onc of the three proposed <br /> borings in the former teepee structure incinerator pit and analyze for the sam e constituents as the <br /> proposed soil samples. <br /> The Addendum is hereby approved provided that a grab groundwater sample is collected from one of the <br /> three proposed borings in the former teepee structure incinerator pit. By 8 August 2002,please submit <br /> an investigation report detailing the field activities and sampling results with a schedule for the submittal <br /> of a health risk assessment. Additional data may be necessary for the HRA based on state and federal <br /> HRA guidelines.
