,l <br /> MCENED <br /> FEB 0 8 2010 <br /> February 5, 2010 ENVIRONMENT HEALTH <br /> Mr. Frank Girardi PERMIT/SERVICES <br /> Senior Registered Environmental Health Specialist <br /> Local Oversight Program <br /> San Joaquin County Environmental Health Department <br /> 600 East Main Street <br /> Stockton, California 95202-3029 f <br /> Subject: Well Destruction Report <br /> Former American Forest Products Facility <br /> SJCEHD Site Code 1882 <br /> 2801 West Lane l <br /> 4 a <br /> Stockton, California 95204 <br /> URS Project No. 17322736 <br /> Dear Mr. Girardi: <br /> INTRODUCTION <br /> This report describes the destruction of the fourteen groundwater monitoring wells located at the <br /> above Site (Site), (Figure 1) This report is provided on behalf of Mr. William Bergvall, <br /> representative of the former American Forest Products Company, by URS Corporation(URS). <br /> The wells were destroyed in accordance with your preliminary "No further Action" letter dated <br /> October 28, 2009. <br /> FIELD ACTIVITES <br /> The fourteen groundwater monitoring wells, DMW-1 through DMW-14,were destroyed <br /> following procedures presented in our work plan dated November 10, 2009 (Attachment 1). That <br /> work plan was prepared in accordance with San Joaquin County Well Standards, County Code <br /> Section 9-1115.6, and information provided by you. The work plan and well destruction permit <br /> application were approved by San Joaquin County on November 24, 2009 (Attachment 2). <br /> Table 1 provides a summary of well construction details. <br /> Six of the wells exhibited residual contamination at the time of the most recent groundwater <br /> monitoring event. In accordance with San Joaquin County requirements, these wells were <br /> destroyed by overdrilling using augers with a minimum diameter equal to the original boring. <br /> Monitoring wells DMW-5, DMW-6, DMW-9, DMW-11, DMW-12, and DMW-14, were <br /> destroyed by overdrilling. The remaining eight wells (DMW-1, DMW-2, DMW-3, DMW-4, <br /> DMW-7, DMW-8, DMW-10, and DMW-13) were destroyed by pressure grouting. <br /> Prior to drilling, all of the well locations were marked and drilling notification provided to <br /> Underground Service Alert as required.
