Mr. Frank Girardi <br /> Senior Reg istercd Environmental health Specialist Page 2 of 2 <br /> Pressure grouting was initiated on January 4, 2010, by Gregg Drilling and Testing of Martinez, <br /> California, under the oversight of URS Geologist, Matt Farris. San Joaquin County oversight was <br /> provided by Lori Duncan on ,January 4, and Frank Girardi on January 5. Pressure grouting was <br /> completed on January 5. At all eight wells grout pressure was maintained at 15 pounds per <br /> square inch (psi) for 5 minutes. <br /> Overdrilling followed by grouting of the remauiing five wells was initiated on January 6, 2010, <br /> and completed January S. Surface completion of the pressure-grouted wells was completed <br /> during the same time by excavating to a depth of five feet below ground surface, leaving <br /> approximately six inches of the PVC casing sticking up within the excavation. The casing was <br /> then topped off with grout which was allowed to overflow the casing, extending to a minimum of <br /> one foot beyond the casing laterally and six inches above the casing vertically, forming a <br /> "mushroom cap". The overdrilling and surface completion activities were observed by Frank <br /> Girardi of San Joaquin County on January 7, 2010. The excavations for the mushroom-capped <br /> wells were backfilled with clean fill on Monday, January 11, 2010. <br /> Soil cuttings and well debris generated during the well destruction has been placed in a lined <br /> rolloff bin for appropriate disposal following receipt of analytical results. <br /> It has been our pleasure to provide this report of well destruction.
