The following is an itemized list of underground storage tank violations that have not <br /> been addressed for 99 SHELL* as of April 27, 2020. <br /> Open violations from August 29, 2019 inspection <br /> Violation #107 -Plot plan/site map not submitted or failed to completely show where monitoring is <br /> performed. <br /> An accurate UST Monitoring Site Plan was not submitted. <br /> -The site plan on CERS is missing the general layout of piping. <br /> A site plan must be submitted identifying the locations where monitoring will be performed. The site plan must show <br /> the general layout of tanks and piping and clearly identify locations of the following equipment, if installed: 1) <br /> monitoring system control panels; 2) in-tank liquid level probes (if used for leak detection); 3) devices monitoring <br /> tank annular spaces or vault; 4) devices monitoring product piping; 5) devices monitoring fill piping; 6) devices <br /> monitoring vent piping; 7) devices monitoring vapor recovery piping; 8) devices monitoring vent/transition sumps; 9) <br /> devices monitoring under-dispenser containment; 10) line leak detectors; and 11) devices monitoring any other <br /> secondary containment areas. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #115- "Designated Underground Storage Tank Operator Identification Form" not submitted within <br /> 30 days. <br /> Erik Rogers is performing designated operator inspections at the facility and is not listed on the submitted <br /> statement of designated operators. The owner or operator shall submit a "Designated Underground Storage Tank <br /> Operator Identification Form" no later than 30 days after any change in the designated UST operator(s). <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload <br /> the notification identifying all the designated operators for this facility. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #121 -Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on 9/6/2018 and an inspection report was issued identifying information to be submitted <br /> to bring this site into compliance. This information was required to be submitted by 10/6/2018. This information has <br /> not been received resulting in a non-compliant status for this facility. An operator that receives an inspection report <br /> shall have 30 days to implement the corrections specified in the inspection report and comply with permit conditions. <br /> The corrective action shall include all of the recommendations made in the report. The operator shall submit a <br /> written response that includes a statement documenting corrective actions taken or proposing corrective actions <br /> which will be taken. Ensure that a written response documenting corrective actions taken or proposed is submitted <br /> and all corrections implemented within 30 days of receiving an inspection report. <br /> Page 1 of 4 <br />
