Modify the site-specific health and safety plan prior to initiating the field work; <br /> b. Submit a groundwater monitoring well abandonment work plan for agency approval <br /> and obtain required permits- prior to start of work. Permits required will include <br /> drilling permits issued by the.SJCEHD.. Notify the SJCEHD prior to the destruction <br /> of the on-site monitoring wells; <br /> l c. Contact the regional utility locating center prior to the destruction of the on-site <br /> monitoring wells; <br /> d. Destroy Monitoring Well Nos. MW-1 and MW-3 by pressure grouting; <br /> e. Destroy monitoring Well Nos. MW-2 and MW-4 by drilling-out the monitoring wells <br /> and then filling the boring with grout; <br /> f. Provide all of the necessary equipment and materials to perform the required services. <br /> This equipment will include. the drill-rig, pressure grouting equipment, and asphalt <br /> t..� cold patch or concrete;. <br /> t g. Re-pave the drilled areas with.an asphalt or concrete finish; and <br /> h.
