The following is an itemized list of underground storage tank violations that have not been addressed for GRANT LINE SHELL* as of May 12 , 2020 . Open violations from May 30, 2019 inspection Violation #201 - Failure to maintain monitoring , maintenance , and follow up action records . Secondary containment results for the test conducted on 5/24/ 18 , 7/5/ 18 , 7/6/ 18 and 7/ 12/18 were not found on site . These results shall be maintained on site for at least three years . Monitoring records include : ( 1 ) date and time of all monitoring or sampling ; (2 ) monitoring equipment calibration and maintenance records ; (3 ) results of any visual observations ; (4) results of sample analysis performed a lab or in the field ; (5 ) logs of all readings of gauges or other monitoring equipment , ground water elevations , or other test results ; (6 ) results of inventory readings and reconciliations . Immediately locate and maintain all secondary containment results for the last three years on site and submit copies to the EHD . ❑ This violation was corrected 11This violation will be corrected by (date) . ❑ Supporting documents included Describe actions taken or will be taken to correct violation : Violation #202 - Failure to maintain the " Facility Employee Training Certificate" on-site ( after October 1 , 2018 ) . On the 4/ 10/19 and 11 /21 / 19 no Designated operator name was provided . On the 4/ 10/ 19 Justin Alejandrino , and Tony Villalobos were trained on 2/ 15/ 19 and the date of assuming responsibility was 2/ 13/ 19 , two days before they were trained . Furthermore , the DO is listing employees in the training forms for which training has been previously provided to in other dates repeatedly. Ensure that the training log is used only for the employees that are train on the date of the training . All site employees must be trained prior to taking responsibility as a facility employee if after October 1 , 2018 , or within 30 days of hire if prior to October 1 , 2018 . The " Facility Employee Training Certificate" shall contain accurate information for all employees trained . Update the " Facility Employee Training Certificate" to contain the accurate information and submit a copy of the updated form to the EHD . ❑ This violation was corrected ❑ This violation will be corrected by ( date) . ❑ Supporting documents included Describe actions taken or will be taken to correct violation : Violation #204 - Current monitoring plan approved by the EHD not found on site .
