The secondary containment spill control equipment for the two 55 gallon drums of used oil have <br /> almost an inch of spilled oil in it. This secondary spill control equipment is about three feet wide, by six <br /> feet long, and about as tall as the two 55 gallon drums contained within it. The secondary spill control <br /> equipment also has approximately five used oil filters laying in the spilled oil. The filters appear dusty <br /> and have a layer of oily debris on top. It cannot be determined how long these filters have been left in <br /> the secondary containment spill control equipment. Facilities that generate up to one ton of used oil <br /> filters per year may store the filters up to one year. Immediately recycle these filters under a bill of lading <br /> and submit a copy of the bill of lading to the EHD by December 30, 2012. <br /> 703. An emergency coordinator and modified contingency plan information is lacking. There must be at <br /> least one emergency coordinator on site or on call to coordinate emergency response measures, and <br /> the following information must be posted by a phone: the name and phone number of the emergency <br /> coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br /> phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an <br /> emergency coordinator and post the required information by a phone. A form is provided that can be <br /> used for this purpose. Submit proof of correction to the EHD by December 30, 2012. <br /> 9999. A routine inspection was conducted on August 6, 2009. Return to Compliance follow up letters <br /> dated September 8, 2009 and November 4, 2009 were sent via certified mail. The Return to Compliance <br /> certification documents which were requested to be received within 30 days of the inspection, have not <br /> been received by the EHD. Per CA HSC section 25185 (c)(3) The operator of the site or facility which <br /> receives an inspection report pursuant to paragraph (2) shall submit a written response to the <br /> department or the local officer or agency authorized to enforce this chapter pursuant to subdivision (a) of <br /> Section 25180 within 60 days of receipt of the inspection report, or within a shorter time as the <br /> department or the local officer or agency may reasonably require, which shall include a statement <br /> documenting corrective actions taken by the operator or proposing corrective actions which will be taken <br /> by the operator, for purposes of compliance with this chapter. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by December 30, 2012. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> Aris Cacapit (209) 468-3453 v �al i <br /> EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
