The following is an itemized list of hazardous waste violations that have not been <br />addressed for ACME SAW & INDUSTRIAL SUPPLY as of April 11, 2016. <br />Violation #101 - Managed a hazardous waste without an identification number. <br />This facility's hazardous waste ID number is inactive. A hazardous waste generator shall not treat, store, dispose of, <br />transport or offer for transportation, hazardous waste without a hazardous waste ID number. Go to the DTSC web si <br />http://www.dtsc.ca.gov/IDManifest/PERMHWID.cfm for the form to reactivate the ID number. Immediately reactivat <br />your State ID number and submit evidence to the EHD. A form was provided at the time of the inspection. <br />Violation #102 - Failed to determine if a waste is a hazardous waste or retain waste analysis on site for 3 <br />years. <br />Sludge accumulated from different tanks in the grinding shops are being used for gardens according to Diane, office <br />manager. Another employee, Mike Volkerts, stated that the sludge are serviced by a company every 6 months, but h <br />does not know the company name. Any person who generates a waste shall determine if the waste is a hazardous <br />waste. Immediately stop disposing sludge in gardens, make a hazardous waste determination for the sludge <br />accumulated from different tanks in the grinding shop, and manage it according the Title 22 hazardous waste <br />regulations. Submit a statement and supporting documentation explaining how this waste was managed. <br />Metal fines generated from metal grinders are being recycled as scrap metal without first determining particle size or <br />hazardous properties. Any person who generates a waste shall determine if the waste is a hazardous waste. <br />Immediately stop recycling metal fines, make a hazardous waste determination for the metal fines, and manage it <br />according to Title 22 hazardous waste regulations. Particles 100 microns or smaller must be handled as hazardous <br />waste if the metal is determined to be a hazardous waste. Submit a statement and supporting documentation <br />explaining how this waste was managed. <br />Violation #103 - No modified contingency plan. <br />An emergency coordinator and modified contingency plan information is lacking. There must be at least one <br />emergency coordinator on site or on call to coordinate emergency response measures, and the following information <br />must be posted by a phone: the name and phone number of the emergency coordinator; location of fire extinguisher., <br />spill control equipment, and if present, fire alarm; and the phone number of the fire department, unless the facility ha! <br />a direct alarm. Immediately appoint an emergency coordinator and post the required information by a phone. A fora <br />is provided that can be used for this purpose.
