The following is an itemized list of hazardous waste violations that have not been addressed for ACME SAW & INDUSTRIAL SUPPLY as of April 11, 2016. Violation #109 - Failed to keep signed copy of manifests from the designated facility for three years. Copies of uniform manifests for the disposal of hazardous waste for used oil, waste gas and oil mixed and used absorbent for 2013 were not found on site. Hazardous waste generators shall retain copies of all manifests signed o by the disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and have them readily available for review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit copies to the EHD. The following manifest numbers were not found on site: a.002363093GBF with the ship date 02/07/14 b.002571396GBF with the ship date 10/17/14 c.00252137GBF with the ship date 3/13/15 Hazardous waste generators shall retain copies of all manifests for three years or until a copy signed off by the destination facility is received. The signed off copy shall be retained and made readily available for review for three years. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with some indication that the generator has not received confirmation of delivery. This information shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. Submi to the EHD a copy of the signed off copy of the manifests or a copy of the information submitted to DTSC. Violation #112 - Failed to keep copies of consolidated manifesting receipts for three years. Copies of hazardous waste disposal records for used oil, waste gas and oil mixed and used absorbent for 2013 were not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and have them readily available for review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, an submit copies to the EHD. Violation #114 - Failed to retain copy of manifest or bill of lading for spent lead acid batteries for 3 years. Bills of lading or manifests for the management of lead acid batteries for the last three years were not found on site. copy of each bill of lading must be kept on site for at least three years. Immediately locate a copy of all bills of lading or manifests for lead acid batteries for the last three years, maintain them on site, and submit copies to the EHD. Violation #601 - Stored hazardous waste onsite greater than 180 days. Observed a 55 -gallon drum of used oil marked with the accumulation start date 7/16/13. Facilities who generate les: than 1000 kg of hazardous waste per month and do not exceed 6000 kg of waste stored on site at any time may stor waste on site up to 180 days. Immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD. Page 2 of 3
