The following is an itemized list of hazardous waste violations that have not been <br />addressed for ACME SAW & INDUSTRIAL SUPPLY as of April 11, 2016. <br />Violation #109 - Failed to keep signed copy of manifests from the designated facility for three years. <br />Copies of uniform manifests for the disposal of hazardous waste for used oil, waste gas and oil mixed and used <br />absorbent for 2013 were not found on site. Hazardous waste generators shall retain copies of all manifests signed o <br />by the disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and have <br />them readily available for review. Immediately locate a copy of all manifests and receipts for the last three years, <br />maintain them on site, and submit copies to the EHD. <br />The following manifest numbers were not found on site: <br />a.002363093GBF with the ship date 02/07/14 <br />b.002571396GBF with the ship date 10/17/14 <br />c.00252137GBF with the ship date 3/13/15 <br />Hazardous waste generators shall retain copies of all manifests for three years or until a copy signed off by the <br />destination facility is received. The signed off copy shall be retained and made readily available for review for three <br />years. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator <br />the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by the <br />initial transporter, the generator shall submit a legible copy of the missing manifest, with some indication that the <br />generator has not received confirmation of delivery. This information shall be submitted to: <br />DTSC Report Repository <br />Generator Information Services Section <br />P.O. Box 806 <br />Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. Submi <br />to the EHD a copy of the signed off copy of the manifests or a copy of the information submitted to DTSC. <br />Violation #112 - Failed to keep copies of consolidated manifesting receipts for three years. <br />Copies of hazardous waste disposal records for used oil, waste gas and oil mixed and used absorbent for 2013 were <br />not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility <br />and all receipts used in a consolidated manifesting procedure on site for three years and have them readily available <br />for review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, an <br />submit copies to the EHD. <br />Violation #114 - Failed to retain copy of manifest or bill of lading for spent lead acid batteries for 3 years. <br />Bills of lading or manifests for the management of lead acid batteries for the last three years were not found on site. <br />copy of each bill of lading must be kept on site for at least three years. Immediately locate a copy of all bills of lading <br />or manifests for lead acid batteries for the last three years, maintain them on site, and submit copies to the EHD. <br />Violation #601 - Stored hazardous waste onsite greater than 180 days. <br />Observed a 55 -gallon drum of used oil marked with the accumulation start date 7/16/13. Facilities who generate les: <br />than 1000 kg of hazardous waste per month and do not exceed 6000 kg of waste stored on site at any time may stor <br />waste on site up to 180 days. Immediately contact a licensed hazardous waste hauler to dispose of this waste under <br />manifest and submit a copy of the manifest to the EHD. <br />Page 2 of 3 <br />
