The following are Class II (Repeat) Violation: <br />404, 605, 610 and the following are Minor Violations - Notice to Comply: 101, 128, 611, 999 <br />101. This facility's EPA ID number is inactive. A hazardous waste generator shall not treat, store, <br />dispose of, transport or offer for transportation, hazardous waste without an EPA ID number. <br />Immediately contact DTSC and reactivate your EPA ID number and submit evidence to the EHD by <br />01/10/2014. Observed owner faxed completed application to DTSC during this inspection. <br />128. Bills of lading for the recycling of used oil filters for the last three years were not found on site. A <br />copy of each bill of lading must be kept on site for at least three years. Immediately locate a copy of all <br />bills of lading for used oil filters for the last three years, maintain them on site, and submit copies to the <br />EHD by 01/10/2014. <br />404 (Repeat). Observed multiple holes drilled onto the top of two 55 -gallon drums of used oil and one <br />55 -gallon drum of waste anti -freeze; therefore these containers could not be closed. All hazardous waste <br />containers shall be closed at all times except when adding or removing waste. Immediately close these <br />containers and ensure all hazardous waste containers are closed when not adding or removing waste, <br />and and submit evidence of correction to the EHD by 01/10/2014. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />TH# FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME D'S CURRENT HOURLY RATE <br />T <br />
