Script 1 - Emergency "Ca16 <br />Code Blue Alert at (location of the medic ergency)" (REPEAT) <br />Script 2 - All Clear "All Clear - Caroline Code Blue - All Clear" (REPEAT) <br />• Call 911, if it is an emergency <br />• If the injury is not an emergency, arrange for the Associate or customer to be provided <br />medical treatment <br />• Secure the area and investigate the incident. Obtain witness statements <br />• Report incidents within 24 hours to the SHC Incident Intake Center at 1-800-727-0121 <br />• Tag, as evidence, any products or displays that may have caused injuries or property damage <br />• Take pictures if possible (Do not photograph the injured/ill person, body parts or body fluids) <br />Note: If the customer has any questions, direct them to call Sedgwick at 866-352-1521 <br />After an Event <br />Refer to the EAP Procedures for reporting requirements. <br />NOTE: <br />OSHA Reporting - All Associate accidents resulting in one or more deaths must be reported by <br />Unit Manager by telephone within 8 hours to OSHA at 1-800-321-6742. In California, accident <br />reporting is required to be reported immediately by telephone or facsimile to the nearest OSHA <br />District Office. <br />Before an Event <br />A fatality is considered a Type 1 Critical Incident. After calling 911 and securing the scene, it is <br />important to make the appropriate notifications to management. <br />
