During an Event <br />1. Call 911 <br />2. Contact a member of management <br />3. Ensure that the area is safe and secure (prevent crowd from gathering at the fatality) <br />4. If a public announcement is needed, use the following script. <br />Caroline Code Blue Alert - Medical Emergency Alert <br />Script 1 - "Caroline Code Blue Alert at (location of the medical emergency)" REPEAT) <br />5. Wait for Law Enforcement <br />6. Once a Type 1 Critical Incident occurs, locations must notify one of the managers listed <br />below. Follow the established communication protocol. Notification must be made either in <br />person or verbally by phone (not a message) as soon as possible but no greater than one <br />hour from the time of the incident. If the District Staff is not available, then notify the next <br />level of management until initial notification has been made. Unless otherwise specified, the <br />communication escalation path is: <br />• District Loss Prevention Manager and District Manager (or equivalent) <br />• Regional LP Director and RVP/GM <br />• Loss Prevention DVP or Director of Corporate Safety <br />• SHC Legal, Public Relations, and Environmental Departments, where applicable <br />Operations <br />7. Immediately report the incident to the SHC Incident Reporting Center, follow instructions as <br />provided <br />8. Box 95407 <br />Customer Inquiries regarding accidents can Hoffman Estates, IL 60195 <br />be directed to 1-866-352-1521 866-352-1521 phone, 888-876-7050 Fax <br />After an Event <br />• Follow instructions of Law Enforcement / Emergency Services <br />• If the fatality is an associate, make the appropriate notification to OSHA <br />• If needed, obtain Crisis Counseling Services by contacting the EAP Hotline 1-800-424-4732 <br />Special Considerations - OSHA Reporting - Associate accidents resulting in one or more <br />deaths must be reported by telephone within 8 hours to OSHA at 1-800-321-6742. In California, <br />accident reporting is required to be reported immediately by telephone or facsimile to the nearest <br />OSHA District Office. <br />0 <br />1.
