These records shall be <br /> maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; <br /> (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of <br /> sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, <br /> ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate <br /> and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. <br /> This is a minor violation. <br /> 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible <br /> on site at all times. Immediately locate and retain a copy on site. <br /> This is a minor violation. <br /> 208 CCR 2715 Designated operator monthly inspection reports not maintained on site for at least 12 months. <br /> No designated operator monthly inspection reports were found on site. Designated operator monthly inspection <br /> reports for the previous twelve months shall be retained on site. Locate and ensure that copies of the previous twelve <br /> months of designated operator monthly inspection reports are maintained on site. Submit copies to the EHD. <br /> This is a minor violation. <br /> 209 CCR 2715(f) Designated operator employee training not performed or log not kept. <br /> Designated operator training records on site were dated 4/12/16. No training records were found for prior to that date. <br /> The designated operator shall train facility employees for which he or she is responsible in the proper operation and <br /> maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br /> the training records to the EHD. <br /> This is a minor violation. <br /> 309 CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. <br /> The Bravo box float and chain in the 5/6 (center) UDC sump failed to stop the flow of product at the dispenser when <br /> tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of <br /> product at the dispenser when it detects a leak. The service technician adjusted the float and verified functionality. <br /> This is a minor violation. <br /> Page 5 of 6 <br />
