Fees for the 2014 annual permit to operate the UST system have not been paid. No <br />person may own or operate an UST unless a permit for its operation has been issued by the local <br />agency to the owner or operator of the UST system. Immediately contact the EHD accounting office to <br />make arrangements for payment to obtain a permit to operate a UST system. <br />Class II violations: (104, 105, 106, 107, 201, 202, 204, 205, 303, 305) <br />104. Financial responsibility documents have not been submitted to the EHD. The most recent <br />financial responsibility documents on record are incomplete and dated April 1, 2013. Current financial <br />responsibility documents are required to be submitted annually. Immediately submit a copy of the <br />financial responsibility documents to the EHD. Alternatively, log into CERS, upload the required <br />information, and submit for review by the EHD. Complete by May 7, 2014. REPEAT VIOLATION <br />105. UST Facility Operating Permit Application and UST Tank Information for the 87 and 91 tanks are <br />not submitted in CERS. Any change of information must be updated in CERS within 30 days of the <br />changes. Immediately log into CERS, update the required information, and submit for review by the <br />EHD. Complete a separate tank form for each tank. <br />106 & 202. Current approved copies of the monitoring and response plans were not found on site. <br />The monitoring and response plans must be current and approved by the EHD. Immediately log into <br />CERS, update the monitoring plan information, and submit for review by the EHD. Complete a <br />monitoring plan record for each tank. Immediately submit a copy of the response plan to the EHD for <br />approval.
