EI Dorado St., Stockton <br />Program: UST <br />SUMMARY OF VIOLATIONS <br />CLASS 1, CLASS II, or MINOR -Notice to Comply) <br />107. A plot plan / monitoring site map is not submitted in CERS. All monitoring programs shall include <br />the location(s), as identified on a plot plan, where the monitoring will be performed. Immediately log into <br />CERS, update the required information, and submit for review by the EHD. <br />201. Monitoring and maintenance records were not found on site for the cold start inspection <br />conducted on September 4, 2013 or the alarm noted in the alarm history for the 87 STP sensor on <br />February 28, 2014. These records shall be kept on site for at least 3 years. Immediately locate and <br />maintain the maintenance and monitoring records on site and submit copies to the EHD by May 7, 2014. <br />202. A copy of the 2014 UST operating permit was not found on site. A copy of the UST operating <br />permit and all attachments and conditions shall be retained at the facility. Immediately obtain a copy of <br />the permit and retain it on site. REPEAT VIOLATION <br />204. The designated operator failed to check and note the correct test dates for the SB989 (triennial <br />secondary containment) testing on the designated operator monthly inspection reports. The test date is <br />documented on the reports as November 1, 2012; according to EHD records, this testing took place on <br />April 10, 2013. During the monthly inspection, the designated operator shall check that all required <br />testing and maintenance for the UST system have been completed and document the dates they were <br />done. Ensure that designated operators performing monthly inspections at this facility are including all <br />of the required information on the reports. <br />205. The April, August, September, October, November, December 2013 and January, February, and <br />March 2014 designated operator monthly inspection reports were not found on site. Alarm history <br />printouts for these months were found in the designated operator binder on site. Designated operator <br />monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure <br />that copies of the previous twelve months of designated operator monthly inspection reports are <br />maintained on site. Submit copies to the EHD by May 7, 2014. REPEAT VIOLATION <br />303. The 87 wrap-around annular sensor was tested and passed at the time of inspection, however the <br />technician could not get the sensor back in the tank. The 87 annular sensor is currently not located at <br />the bottom of the tank to detect a leak at the earliest opportunity. Monitoring equipment shall be <br />maintained to be able to detect a leak at the earliest possible opportunity.
